Guidelines for CMII Students and Other Users
Checkout for Students in CMII Classes
CMII Students may checkout equipment and resources with the approval of a CMII Faculty member. Approvals are most often provided as a part of academic course work or research projects. The standard duration for these checkouts is no more than five business days. Any other duration must be approved by CMII Faculty or Leadership.
The faculty or staff member must authorize the checkout for one or more students by completing this Checkout Authorization Request. Faculty, staff and students should allow up to 3 business days for this authorization to be processed and confirmed. Please note that the reservations are not guaranteed until the requestor has received an email confirmation.
Checkout for All Other Georgia State Students
Any GSU student may check out equipment from the CMII’s General Checkout Inventory for daily use in the CMII building. The equipment must remain in the building and be returned within 30 min of the close of business. Checkout hours are M-F, 10 am to 5 pm. CMII Faculty or Leadership must approve other location or duration uses.
All Others
If you have a request to checkout equipment that does not match the scenarios above please contact us and describe the nature of your request.
Check Out Terms
Responsibilities:
- The borrower agrees they are personally and solely responsible for any damage, loss or theft of equipment checked out to them.
- Before signing the checkout form, the borrower must check the equipment for damage and missing parts.
- When returning equipment, the borrower must notify CMII of any problems.
- Returns must be made during regular business hours, at least 30 minutes before closing.
- The borrower represents that they have the necessary skills to use this equipment properly. Some checkouts require training or documentation to ensure borrowers can use the equipment properly. CMII must approve any exceptions.
Fees & Penalties:
- If equipment is damaged, the borrower will be charged for repair costs; if it is lost or stolen, the borrower will be charged the current retail cost.
- CMII has up to 5 business days to inspect the returned equipment and notify the Borrower of any damages. The borrower has up to 30 days from notification to dispute any damage charges.
- The borrower must return the equipment on time and pay any damage or loss fees. Failure to do so may result in:
- Garnished wages for faculty or staff.
- Prevention of borrower registration, graduation or issuance of a transcript for students.
- Additional fees and suspension of privileges for affiliates.
- Late, lost and/or administrative fees.
- Legal action. In this case, the borrower agrees to pay all collection fees, attorney fees or other expenses due to Georgia State as a result of violation of this agreement.
Usage:
- The borrower understands that CMII equipment may not be used for personal financial gain or commercial purposes.
- Priority for borrowing is given to academic or instructional use, but borrowers may use equipment for any non-profit purpose.
Data Storage:
- The borrower grants CMII permission to store their name, Panther ID number, phone number, email address and checkout record. The borrower must notify CMII of any changes or updates to this information.
Checkout Privileges:
- CMII reserves the right to deny checkout privileges.
Indemnification:
- The borrower agrees to release, hold harmless and indemnify the Board of Regents of the University System of Georgia and its officers, agents, servants and employees from all claims or liability for property damage, personal injury or death arising out of the borrower's use of the equipment or their permission (express or implied) to use it.
